Group Therapy

6 hours per day, 5 days per week. Most people are skeptical about group therapy at first – the idea of sitting in a circle talking about feelings may sound unbearable. But what happens is you realize you’re not alone and you start building relationships with people who understand what you’re going through. We keep groups small – 8 clients – and our curriculum is clinically structured around the specific patterns and challenges our clients face. We focus the work on the thoughts, behaviors, and situations that keep people stuck.

Individual Therapy

Weekly one-on-one sessions, typically 60-90 minutes, with a master’s-level therapist. Addiction doesn’t exist in a vacuum – there’s always something underneath driving it. Trauma you’ve never processed. Anxiety you’ve been self-medicating for years. Beliefs about yourself that have been running in the background your entire life. Individual therapy is where you get underneath the substance use and start working on what’s actually keeping you stuck. It’s confidential, it’s tailored to you, and it goes as far as you’re ready to take it.

Psychiatry

Our in-house psychiatric provider conducts a full assessment when you start, manages medication adjustments as needed, and monitors your progress throughout the program. This isn’t a fifteen-minute appointment with someone who barely knows you. It’s integrated care – your psychiatrist talks to your therapist, knows what’s happening in group, and adjusts treatment based on how you’re actually doing. The goal is clinical stability that supports your recovery, not just symptom management.

Recovery Coaching

Weekly one-on-one sessions focused on building the recovery infrastructure that continues after treatment ends. Your Recovery Coach isn’t your therapist – they’re the person who helps you translate what you’re learning in clinical sessions into actual life – and in real-time. They help you set realistic goals, build accountability structures, and connect with recovery communities outside of Lighthouse. PHP gives you clinical tools – your Recovery Coach helps you put them into practice.

Recovery doesn’t happen in a vacuum. Lighthouse combines clinical care, psychiatric support, life-skills training, family work, and continued support into one integrated program designed to address every aspect of a client’s recovery. Whether someone is in our outpatient programs or our Extended Care platform –  they’re getting holistic care –  with every component working together to build lasting recovery and an independent life.

Client-centered philosophy

We treat people, not diagnoses. Every client’s journey begins with a comprehensive assessment, and every treatment plan is built around their unique history, their personal goals, and the level of support they actually need. Our approach is progress-based and shame-free, focused on understanding and addressing the underlying causes of addiction rather than simply managing symptoms. Recovery is deeply personal, and we believe treatment should be too.

Accountability

We work with clients who are ready to do the work. That means high accountability – required meeting attendance, regular drug testing, weekly case management, and clear expectations throughout the process. For some of our Extended Care clients, the decision to come to treatment was made by their family, not themselves, and that’s okay. We meet clients where they are and help them find their own reasons to stay, because lasting recovery requires willingness – and our job is to create the conditions where that willingness can take root and grow.

Clinical Excellence

Our clinical team includes masters-level clinicians with real expertise in trauma, attachment, family systems, and co-occurring disorders. Groups are capped at eight clients – half the industry average – because meaningful therapeutic work requires attention that simply isn’t possible in larger settings. The curriculum is custom-designed in-house, and we maintain JCAHO accreditation, the highest standard in healthcare. Every piece of our clinical infrastructure exists for one reason: to deliver the kind of care that actually produces lasting change.

What is a Partial Hospitalization Program (PHP)?

Partial Hospitalization Programming (PHP) is the most structured level of outpatient treatment – one step below residential care. You attend full-day programming (typically 6 hours per day, 5 days per week) that includes multiple therapy groups, individual therapy, psychiatric care, and recovery coaching, then return home each evening. It’s called “partial hospitalization” because it provides clinical intensity comparable to inpatient care without requiring you to live on-site.

Why does PHP work?

PHP works because it combines clinical depth with real-world practice. You’re getting serious therapeutic intervention during the day – the kind of structure that creates rapid stabilization – then immediately practicing those skills in your actual life every evening and weekend. You’re not isolated from reality, but you’re also not left to figure things out on your own with minimal support. The daily accountability and psychiatric oversight address acute issues quickly, while the return home each night forces you to apply what you’re learning in real time.

How is PHP different from residential treatment?

Residential treatment requires you to live on-site 24/7, which provides maximum structure but also removes you from your normal life entirely. PHP gives you comparable clinical intensity during the day but allows you to go home at night. You’re building recovery skills while still navigating your actual environment – your home, your relationships, your triggers. For many people, this makes the transition out of treatment less jarring because you’ve been practicing recovery in real-world conditions from day one.

How is PHP different from IOP?

IOP (Intensive Outpatient Program) typically involves 9 hours per week – three therapy groups, one individual session, and recovery coaching. PHP is 30+ hours per week of full-day programming with psychiatric oversight. The difference isn’t just time – it’s clinical intensity. PHP is for people who need more structure, more frequent check-ins, and psychiatric monitoring that IOP can’t provide. If you’re in crisis, recently completed residential treatment, or tried IOP without success, PHP offers the higher level of care you need.

Who is PHP for?

PHP is for people who need serious clinical structure but are stable enough to sleep at home. That might mean you just finished residential treatment and aren’t ready for weekly outpatient. It might mean you tried IOP and it wasn’t enough. It might mean you’re in active crisis – using daily, unable to stop on your own – but don’t require 24/7 supervision. PHP is also appropriate if you have co-occurring mental health issues that need psychiatric management alongside addiction treatment.

Why is group therapy important?

Addiction thrives in isolation and shame. Group therapy breaks both. You realize you’re not uniquely broken when you hear your own thoughts come out of someone else’s mouth. You learn to be honest in front of people who actually understand what you’re dealing with. You build relationships with others in recovery that extend beyond treatment. Group also provides real-time feedback and accountability in ways individual therapy can’t – you can’t hide patterns when other people in the room are watching them play out.

How long does PHP typically last?

Most people are in PHP for 2-4 weeks, though length varies based on clinical need and progress. Some people step down to IOP after a couple of weeks. Others need a full month or longer if they’re dealing with complex trauma, severe mental health conditions, or ongoing instability. Your treatment team evaluates your progress weekly and adjusts the plan as needed.

What does a typical day in PHP look like?

You arrive in the morning and participate in structured programming until mid-afternoon – typically 5-6 hours. That includes multiple therapy groups (process groups, skills-based groups, psychoeducation), individual therapy sessions with your clinician, meetings with your recovery coach, and check-ins with the psychiatric provider as needed. There are breaks for lunch and time between sessions, but the day is full. You’re not sitting around waiting – you’re actively engaged in therapeutic work.

Can I work while in PHP?

Most people cannot work full-time during PHP because the program requires 6 hours per day, 5 days per week. Some people take medical leave. Some work part-time in the evenings if their schedule allows. Some are students who pause school temporarily. PHP is designed for people who need to prioritize treatment over other obligations during an acute period. If maintaining full-time work is non-negotiable, IOP might be a better fit.

Do I have to live in a sober living home during PHP?

No. Many people live at home with family or in their own apartments. Some live in our Extended Care residences or other sober living homes because they need additional structure and accountability outside of treatment hours. Your living situation depends on your stability, support system, and what your clinical team recommends. If your home environment is unsafe or actively triggering, we’ll help you find alternative housing.

What happens if I relapse during PHP?

Relapse doesn’t mean automatic discharge, but it does require an honest conversation with your treatment team about what happened and whether PHP is still the appropriate level of care. If you used but you’re honest about it, engaged in the process, and willing to address what led to the relapse, you stay in the program. If you’re using repeatedly and not engaging, or if it’s clear you need a higher level of care like residential treatment, we’ll discuss next steps. The goal is to keep you safe and match you with the level of structure you actually need.

Is PHP covered by insurance?

Most commercial insurance plans cover PHP as it’s a recognized level of care. Coverage varies by plan – some cover it fully, some require copays or coinsurance. We verify benefits before you start and let you know exactly what your financial responsibility will be. If insurance denies coverage or you don’t have insurance, we can discuss private pay options and payment plans.

What comes after PHP?

Most people step down to IOP after completing PHP, then transition to standard outpatient therapy and recovery community involvement. Some people continue living in Extended Care housing while attending IOP. The step-down is gradual and planned – you don’t go from full-day treatment to nothing. Your treatment team works with you to build a discharge plan that includes ongoing therapy, psychiatry if needed, recovery meetings, and accountability structures.

Do I need a referral to start PHP?

You don’t need a referral, but we do require a clinical assessment before admission to determine if PHP is the right level of care. If you call us directly, we’ll schedule an intake assessment. If you’re being referred by another treatment provider, hospital, or physician, they’ll typically send records and we’ll coordinate the transition.

What if I have co-occurring mental health issues?

PHP is specifically designed for people with co-occurring disorders – depression, anxiety, PTSD, ADHD, bipolar disorder, or other mental health conditions alongside addiction. Our psychiatric provider conducts a full evaluation, manages medications, and monitors your mental health throughout treatment. You can’t successfully treat addiction without addressing underlying psychiatric issues, which is why integrated care is built into our PHP model.
