Clinical Programming

Clients progress through 6+ months of clinical programs – Partial Hospitalization, Intensive Outpatient, and Supportive Outpatient. The programming adjusts in real-time with the client, addressing addiction, trauma, mental health conditions, and practical life skills. Group therapy, individual therapy, and psychiatric care integrated throughout. The clinical work doesn’t stop when symptoms improve – it continues until the client proves they can maintain progress independently.

Residential Living

Clients live in 1 of 4 residences in Preston Hollow, with a maximum of 8 clients per home. Each house has a live-in house manager and we maintain close to a 2-to-1 client-to-staff ratio. Clients live with peers working toward the same goals, participate in regular drug testing, and have meals and transportation provided. The residential component provides the accountability that makes clinical work sustainable – giving clients a place to practice independence while still having immediate support when they need it.

Family Case Management

Addiction affects the entire family, and sustainable recovery requires addressing the family dynamics that either support or undermine it. Each client has a dedicated case manager who coordinates between clinical staff, house managers, psychiatry, and the family while providing active support for parents navigating their own healing. Over weekly calls and in family therapy, we work throughout the process to ensure families are not just informed – they’re equipped.

Life Skills Training

Our life skills curriculum covers everything from budgeting to resume building, job searching, and maintaining employment. Clients learn house skills – cooking, cleaning, managing their space – alongside the practical work of navigating school, work, or volunteer commitments. For clients who have spent years in crisis mode or simply never launched, this is where they learn to function as independent adults – and they graduate ready to handle the responsibilities of adult life.

Comprehensive
Approach

Recovery doesn’t happen in a vacuum. Lighthouse combines clinical care, psychiatric support, life-skills training, family work, and continued support into one integrated program designed to address every aspect of a client’s recovery. Whether someone is in our outpatient programs or our Extended Care platform –  they’re getting holistic care –  with every component working together to build lasting recovery and an independent life.

Client-centered philosophy

We treat people, not diagnoses. Every client’s journey begins with a comprehensive assessment, and every treatment plan is built around their unique history, their personal goals, and the level of support they actually need. Our approach is progress-based and shame-free, focused on understanding and addressing the underlying causes of addiction rather than simply managing symptoms. Recovery is deeply personal, and we believe treatment should be too.

Accountability

We work with clients who are ready to do the work. That means high accountability – required meeting attendance, regular drug testing, weekly case management, and clear expectations throughout the process. For some of our Extended Care clients, the decision to come to treatment was made by their family, not themselves, and that’s okay. We meet clients where they are and help them find their own reasons to stay, because lasting recovery requires willingness – and our job is to create the conditions where that willingness can take root and grow.

Clinical Excellence

Our clinical team includes masters-level clinicians with real expertise in trauma, attachment, family systems, and co-occurring disorders. Groups are capped at eight clients – half the industry average – because meaningful therapeutic work requires attention that simply isn’t possible in larger settings. The curriculum is custom-designed in-house, and we maintain JCAHO accreditation, the highest standard in healthcare. Every piece of our clinical infrastructure exists for one reason: to deliver the kind of care that actually produces lasting change.

How long is the Extended Care program?

The average length of stay is 8 months, though some clients stay longer depending on their progress and clinical needs. We don’t rush people out, and we don’t hold them longer than necessary. The program ends when the client is ready – when they’re setting their own goals, following through on commitments, and demonstrating they can maintain progress independently.

How is Extended Care different from a 30-day program?

Thirty days is enough time to stabilize someone. It’s rarely enough time to change them. Extended Care provides the time required to address what’s actually driving the addiction – trauma, mental health conditions, family dynamics, undeveloped life skills – while giving clients a structured environment to practice living differently. By the time they graduate, they’ve been doing the work long enough that it’s become habit.

Who is Extended Care for?

It’s for young men who need more than short-term treatment can provide. That might mean they’ve been through programs before that didn’t stick, or they’re dealing with co-occurring mental health issues that require sustained clinical attention. It might mean they’ve never learned to function as an independent adult. Some clients come after multiple treatment attempts. Others are going to treatment for the first time but need a level of structure and accountability that a short-term program can’t offer.

What does a typical day look like?

Days are structured and full. Clients participate in clinical programming – group therapy, individual therapy, and psychiatric care as needed – alongside house responsibilities, life skills work, and outside commitments like jobs, school, or volunteer work depending on where they are in the program. Evenings include recovery meetings, house activities, and time with peers. The structure is intentional and adjusts as clients progress.

Where do clients live?

Clients live in one of four residences in the Preston Hollow neighborhood of Dallas. Each home houses a maximum of 8 clients and has a live-in house manager. The homes are fully furnished, and meals and transportation are provided. We maintain close to a 2-to-1 client-to-staff ratio across the program.

Can my son work or go to school while in the program?

Yes – eventually. Early in the program, the focus is on clinical work and establishing stability. As clients progress and demonstrate accountability, they begin taking on outside commitments like part-time work, school, or volunteer positions. This is an important part of the program – learning to manage real responsibilities while still having support.

How involved will our family be?

Very involved. Each client has a dedicated case manager who serves as the primary point of contact for the family. Parents receive weekly updates and participate in family therapy. We also work with families on their own dynamics – helping them understand patterns that may have developed over years of crisis and equipping them to support their son in a way that doesn’t enable old behaviors.

Can parents visit?

Yes. We encourage family visits and work with families to schedule them at appropriate times in the client’s treatment. Early in the program, we may recommend limiting visits while the client establishes stability. As they progress, visits become more frequent and eventually include off-campus time together.

What clinical programming is included?

Clients progress through multiple levels of clinical care over the course of the program – Partial Hospitalization (PHP), Intensive Outpatient (IOP), and Supportive Outpatient. Programming includes group therapy, individual therapy with a master’s-level clinician, family therapy, and psychiatric care. The level of intensity adjusts based on where the client is in their recovery.

How do you handle relapse?

Relapse doesn’t mean automatic discharge. It means an honest conversation about what happened and a reassessment of the treatment plan. We look at what led to the relapse, whether the current level of care is appropriate, and what needs to change going forward. The goal is to address it directly and use it as information – not to punish the client for struggling.

What happens after the program ends?

Clients graduate with a continuing care plan that includes ongoing recovery support, connection to community resources, and clear next steps for housing, employment, or school. Many clients stay connected to the Lighthouse alumni community. We also offer a Recovery Coaching program for clients and families who want continued accountability after graduation.

Does insurance cover Extended Care?

Insurance typically covers the clinical components of the program – PHP, IOP, and individual therapy. The residential living portion is private pay. We work with families to verify benefits and create a financial plan before admission. Our team can walk you through what to expect based on your specific coverage.

What if my son doesn’t want to be there?

Many clients arrive ambivalent or resistant. That’s normal. The structure of the program creates enough stability for the clinical work to begin, and most clients shift over time as they start to experience progress. Our staff are experienced in working with clients who aren’t fully bought in yet. Willingness often develops through the process rather than before it.

How do you measure progress?

We track clinical progress through regular assessments and treatment plan reviews. But we’re also watching for the things that matter in real life – whether a client follows through on commitments, how he handles conflict, whether he’s honest when no one’s forcing him to be. The real measure is whether he can function independently by the time he leaves.
